Items you need to provide when your children start school at Harvey Oaks:
*Up-to-date Immunization Records
*New Physical if out-of-state - A waiver in the registration packet can be signed and used until a new physician can be found for the physical.
*Original Birth Certificate - (with raised seal)
*Previous school address - (for transfer of records)
*School Records - (report cards, assessments)
Copies of Individualized Education Plans for students receiving Special Education Services A copy of the Purchase Agreement if the house is not closing before the starting school.
